The information requested on this form can be completed by a carer, but only those with parental responsibility can sign the consent (NB this may or may/not include a foster carer)

    We're very excited for SUMMER '22 week which will take place from Sunday, 24th July to Sunday, 31st July at:

    • Restore Community Church, Loughton
    • New River Baptist Church, Islington 
    • The Well Community Church, Camberwell
    • 'East & West' with the team going to the Isle of Dogs (Sunday, 24th -Tuesday, 26th) and Hounslow (Wednesday, 27th-Sunday, 31st)

    This project is facilited by iNet and REACH London.

    Please also let us know your tetanus jab status as we plan to be outdoors working with tools. There is an option below for you to tick whether if you've received your final dose at age 14 or not. For more information on this, click here.

    COSTS/DONATION 

    We welcome a contribution of £20 towards the costs of a SUMMER '22 T-shirt, training, accommodation and food. But please do not let that stop you from joining us! We value your participation a lot more than this contribution.   • MEDICAL CONSENT FORM

    Whenever medical advice or treatment is needed, the assistance of onsite medical teams, a GP or A&E Department should be sought. The Children Act 1989 allows a doctor to provide any necessary treatment by doing ‘whatever is reasonable in all the circumstances of the case for the purpose of safeguarding or promoting the child’s welfare. However, the parent/carer should be contacted and advised of the situation as soon as possible. It is important, however, that we obtain in advance from parents/carers:


    1. All necessary information concerning the child/young person’s health, allergies, medication etc

    2. Written agreement as follows:


    I understand:

    • My child will receive medication as instructed before or during the event
    • Every effort will be made to contact me as soon as possible should my child become ill or have an accident
    • My child will be given medical/dental treatment as necessary
