Check When Acknowledged 

For all employees or Sub-contractors working for The Berg Group LLC new to
that specific jobsite. To be completed by all site supervision and employee prior to beginning work.

First violation - Verbal warning/documented

Second violation - Verbal & written warning

Third violation - Verbal & written warning and three-day suspension without pay

Fourth violation - Employee discharge from company

**The Berg Group reserves the right to bypass 1-3 violation procedures and proceed directly to discipline for the fourth violation.

This is to acknowledge that I have completed new employee Safety orientation and understand that failure to comply with the Safety policies outlined in this orientation checklist may be grounds for disciplinary action, up to and including termination.
