The Sweet Squad Application Form

The Sweet Squad Jax chapter is a year long girl leadership development experience in which up to 20 elite girls across Jacksonville, Florida will be selected to participate.

Program Location: Springfield (Exact location TBA)

Program Cost: $25 Application fee +  1 case of Water.   Water is due at orientation.  Food/transporation covered during workshops. Students are responsible for getting to and from the workshops)

Eligibility:

The Sweet Squad is looking for smart, driven, team-oriented, resourceful, and entrepreneurial-spirited individuals. Most importantly, we are looking for dedicated students; The Sweet Squad is an intensive program that requires a 1-2 hour monthly commitment outside of the in-person weekend sessions.

Requirements:
• Girl age 7yo to 18yo
• Willing to complete 50 hrs of Community Service Annually
• Applicants must be full of determination, able to work well in teams, not afraid to take risks, display confidence, crave learning, understand failure is part of the game and be passionate about finding ways to improve their communities/city.

Please note: All students are allowed to miss a total of 2 workshops. The sessions involve a high level of group work, ideating, and team development activities.

Thank you for agreeing to allow your child to participate in the upcoming Sweet Squad Programming (“Events”).  We are excited to have your child take part in our programming.

The Event will be distributed to a broad audience through various forms of media.  The Sweet Squad and its designees and sponsors intend to promote and exploit the Event through advertisements, promotions, and media featuring photographs and footage of the Event, as well as interviews with your child and other participants.

By participating in the Event, you agree that The Sweet Squad and its member teams, and their sponsors and affiliates, and collectively, the “Released Parties” may use (and authorize others to use) the video capture of the programing and events, as well as your child’s name and likeness, in advertisements, promotions, in any and all materials, and in any and all media in connection with the programing and events, including the media distribution and promotion thereof. By participating with the Sweet Squad, you consent to the use of your child’s name and likeness and you also agree to release and waive any claim you or your child may have against the Released Parties and any of their respective related entities.

 Thank you again for your child’s participation. By registering your child to participate with the Sweet Squad, you are acknowledging this release and the consent to and acknowledgement of the above.

CODE OF CONDUCT

You will behave in a way as to create a safe and supportive environment for all Sweet Squad participants (including Moderators, Speakers, and Attendees).

 You will not engage in disruptive speech or behavior or otherwise interfere with the virtual event or other individuals’ participation in the event.

 You will not engage in any form of discrimination, bullying or harassment of any kind, including online, emotional or sexual harassment; hate speech; or rude behavior of any kind.

 You will comply with the instructions of the moderator and any Sweet Squad event staff.

 You will comply with all applicable laws.

 All Sweet Squad programs are open and inclusive of any youth regardless of race, religion, ethnicity, political belief, socioeconomic status, sexuality, or physical ability or other identity. Respect, tolerance and understanding of others’ backgrounds and beliefs must be exemplified at all times.

 Please note that we may be filming and taking photos during this event. By attending, you consent to such recording and its release, publication, exhibition, or reproduction to be used for outreach, social media, or other promotional purposes.
