Match Alteration Request – 2022 SL Competitions

For Senior League competition matches only
This form is for match alteration requests for the 2022 SL season. All requests require the details of the match as currently scheduled and the requested details for the match.

Requests must be submitted a minimum of seven (7) days in advance. Confirmation for the request from the opposition club will be required before any matches can be altered.
