Once your appointment is scheduled, you will receive a link to our Advanced MD Patient Portal to fill out the required patient forms. If you are not able to fill out your forms, please come to the office 30-45 minutes earlier to complete the forms.
What to bring with you to your appointment:
1) Insurance Cards
2) Photo ID
3) Current medication bottles OR printout from the pharmacy
There is a missed appointment/late cancellation fee of $50 if you do not cancel at least 24 hours prior to your appointment time.
The first appointment is for evaluation and it is not a guarantee of ongoing care or medications.
NOTICE: All new patients are required to have a credit card on file for protection of no-shows/late cancellations. Our office does a courtesy call on behalf of you to your insurance company to verify your benefits such as copay, coinsurance, deductible, out-of-pocket and accumulations. However, occasionally they may deny payments for reasons such as authorization issues, incorrect benefit information (deductible, copays, etc.), coordination of benefits, change of benefits due to the new month, etc. You would be ultimately responsible for all services rendered for which a charge may be associated.
