Moonflower Student Registration Form

Fill out the form carefully for registration. The eight-week series begins Nov. 22-Jan.3, 2025. Class will take place on Saturday mornings at 10 a.m., at Studio Soul, 1216 16th St. West #33. Cost is $100 for the beginner's class and must be paid IN FULL CASH ONLY on July 7, 2025. All payments are nonrefundable. Students under 18 must have parent's permission via instructor. Music with explicit lyrics may be used during session.
REFUND POLICY

Please note that class registration fees are non-refundable and non-transferable. All payments must be made at the first class, cash only.

LIABILITY WAIVER

Moonflower Belly Dance, instructor and owner Charity Dewing, the City of Billings, Studio Soul, Craft Local, and their associates accept no responsibility for personal injury that may take place during classes or while arriving at or leaving the facilities.

I hereby release, discharge, covenant not to sue, and agree to indemnify and save and hold harmless each of the releasees from all liability claims, demands, losses, or damages on my account caused or alleged to be caused, in whole or in part by the negligence of the releasee or otherwise, including negligent rescue operations. I further agree that if, despite this release, I, or anyone on my behalf makes a claim against any of the releasees named above, I will indemnify, save, and hold harmless each of the releasees from any litigation expenses, attorney fees, loss liability, damage, or any cost that may occur as the result of any such claim.
