-We will accept new patients at age 6 and up.-Children are not allowed in the operatory rooms while you are receiving treatment.-Children are not to be left unattended in the waiting area while you are receiving treatment.-No one under 16 years old is allowed to be dropped off without a responsible parent/guardian.-College students or elderly parents who are unable to make financial and/or treatment decisions should bring their parents/guardians/caretakers with them to their first appointment.-The waiting area is reserved for patients with appointments. Due to limited space in the waiting room we ask patients with appointments to please limit the number of family/friends at the time of appointment.
-You must call or leave a message 24 hours before your appointment to cancel without being charged a cancellation or broken appointment fee.
-You will be charged a $35 fee for broken appointment(s)-Multiple broken appointments may result in dismissal from the practice
-We ask that you reschedule your appointment if you are sick with the flu or any other viral diseases.
-We ask that you reschedule your appointment if you are unable to pay for your visit.
-If you are more than 10 minutes late, your appointment may have to be rescheduled.
-When you arrive, check-in with the receptionist and alert them of any changes in your personalinformation: name changes, address, phone number, insurance, or medical history.
-We do not offer payment plans. If you choose to, we can submit your application to Care Credit.
-Returned checks will be charged a $25 fee in addition to any previous balance.
-Balances from previous visit’s copays, deductibles, balances from paid claims, and denied services must be paid IN FULL before any future appointments are scheduled.
-You will be required to pick up your records in person and pay a $10 fee.
