Quinn's Tattoo Booking Form

Please read all of the information below before submitting this form.
- LOCATION: I currently tattoo out of ShapeWorld in Brooklyn. The studio is conveniently located in between the Morgan and Jefferson L stops and is wheelchair accessible. The studio is home to 4-5 other tattoo artists.
- Clients must be 18 or older
- When booking I ask for a $50 non-refundable deposit (paid to Zelle #518-526-0904) in order to reserve your time slot. This deposit counts towards the final cost of the tattoo.
- For payment on the day of the appointment, pls bring cash. There is also an atm across the street from the studio.
- PRICING: Flash designs are priced depending upon scale and density. Most designs fall between $150-$450
- Please do not schedule anything which is time sensitive directly after the tattoo appointment. Appointments can often take more or less time than expected but there is no reason to rush the process!
Once a booking form is submitted I will then email you with the available time slots. Once a deposit has been made that time slot is yours! If for whatever reason you cannot make an appointment please contact me within 3 days, that way I can try to slide you into another open time slot/fill the one that is opening up.
