Church Facility Request

Thank you for your interest in utilizing the facilities at our Crossroads Church of Shamong home. We believe our facilities are a tool for the sharing of good news of Jesus with people in our community.

Usage of the facilities is at the discretion of the church leadership and we reserve the right to grant or deny access based upon the vision and values of our church. Please note that this form is a REQUEST that must be approved via e-mail before permission is granted to use the facilities. In many cases with outside organizations, this will require a Certificate of Insurance.

For any Church Facility Request, we ask you to identify an event "host" who is responsible for the follow through of the info below. This person must be at least 21 years of age.

Please note that due to the costs of utilities and cleaning/upkeep with the building, we may ask for a suggested donation if your event requires it. Typical suggestion donation rates assume up to 4 hours of useage (see room specifics below). These donations will help us defer the costs of utilities and a possible cleaning fee for larger or longer events. We will be in touch with you via email about this if necessary.

I acknowledge that requests are not approved until a response email states as much. I also acknowledge that my group's "Host" must be present for the whole time your group is there. Also, once your event is completed, all tables and chairs should be wiped down and returned, floors swept, spills wiped up, and trash cans emptied and put into the back dumpster. *
