Accompanied minor

All children attending the Lion Cubs Playgroup need to be accompanied by one (1) adult per child. The child will be under the direct supervision and care of the responsible adult. Anglo strongly recommends for the benefit of the child and parent, that children are accompanied by either a parent or grandparent and not by a nanny

No refund of fees

In the event where the child is sick, hospitalised or under the instructions of the doctor/s to stay away from this Programme, a makeup session will be held for the child on an alternate day where a vacancy is available. The parents will need to provide a medical certificate in order to qualify for a makeup day. In the event that Anglo cancels a class, an alternative make up day will be scheduled for the child.

Confirmation of seat

The Lion Cubs Playgroup works on a first come first served basis. A spot will only be deemed reserved once payment has been made.
Consent

By submitting this form, I understand the terms and conditions and accept that I, or the assigned adult, am/are solely responsible for the supervision and well-being of my child while my child is attending the Anglo Lion Cubs Playgroup.

I acknowledge that I cannot hold Anglo Singapore International School responsible in anyway during the time that my child is on Anglo Singapore International Schools premises as the child is under my/the responsible adult’s direct supervision.

In the event of any injury my child may sustain while attending the Lion Cubs Playgroup, I will not hold the school and S.B.I.E. Co., Ltd. responsible or any of its members liable. I also acknowledge that I will be liable for any medical expenses for my child.

I agree to allow Anglo Singapore International school to use photographs and videos of the responsible adult and child taken during the Anglo Lion Cub Playgroup sessions for publicity and marketing purposes.
