  • Note:

Our standard adoption fee is $500 which includes all of their vetting. Unless stated otherwise you can expect a dog to have our standard adoption fee.
  • This questionnaire is designed to help R.A.V.E and yourself determine if a specific pup is the right one for you and your household. ALL adopting parties are required to complete this form prior to adoption.

Please note these are NOT trick questions. We are one of the most open minded rescues you are going to find. With that being said we hold our adopters to their answers. So while we might make an exception if something is brought to our attention, we will not if we find out ourselves.

R.A V.E reserves the right to deny any adoption based on answers to this questionnaire and/or interviews prior to adoptions!

R.A.V.E is an all-volunteer, animal welfare organization dedicated to the rescue and placement of stray, neglected and abused animals. R.A.V.E is completely funded out of pocket and through the occasional public donation.

To be considered for adoption you must:
    • Be at least 21 years old
    • Have knowledge & consent of all adults living in your household
    • Have a valid government issued photo ID
    • Have landlords’ consent (if renting) to bring an dog onto the property.
    • Understand that R.A.V.E reserves the right to refuse any adoption application for any reason and that all animals require a home visit before adoptions are granted.
    • Dogs in the home must be spayed or neutered to be considered for an adoption. (Exceptions can be made if we are aware of the situation at hand)

  • Veterinary Information

  • Potential adopters who already have a companion animal or who have had a companion animal must consent to allow a R.A.V.E representative to perform a check with your current veterinarian clinic.  This is usually done via a phone call to the clinic. R.A.V.E will be checking to ensure that your current animal(s) is up to date on all vaccines and heartworm/flea preventative.

By signing below, I consent and agree that a representative of R.A.V.E will perform a veterinarian check to ensure that my current animals are up to date on recommended vaccines.

  • Lastly

  • I understand that I assume full responsibility for the welfare of this pet from the date of adoption.

Because so many rescued animals have unknown medical histories, a quarantine period is recommended, in some cases, for animals that have been in rescue for less than two weeks. The date the animal arrived into the rescue will be disclosed upon request.

No Refunded Adoption Fees -- No cash refunds or charge credits will be given for returned animals after the adoption. Please think about the adoption decision carefully and thoroughly! Failed placements happen so please remember that!

Ownership Agreement – As the new owner, you agree to take full responsibility for the care and wellbeing of this pet for its lifetime. Once the adoption is finalized you will be responsible for all medical bills and decisions.

You understand that if at any time R.A.V.E feels the animals wellbeing is in jeopardy due to negligence or lack of care we will take the necessary steps to remove the animal from your care.

I have read, understand and agree to abide by the above-mentioned policies.
