SBJJ Participant Waiver

I, as a participant or parent/guardian of a participant, in consideration of the training being offered by Piasa Strategics, LLC. (DBA Strategic BJJ), am aware in making this agreement that certain elements of the training are physically demanding and potentially dangerous, and with this knowledge agree to indemnify and hold harmless from all losses caused by accident or injury the instructor, their assistants, or any third parties who may be instructors, participants or attendees of the same class or seminar, in the event that the participant or the said third party is injured in any way during the performance and execution of techniques or instruction provided in this training.

Participant/Guardian agrees that the terms hereof shall serve as a release and assumption of risk for their heirs, executors and administrators, and for all members of their family, including any minors.Participant/Guardian will take care to treat the training facilities, and property utilized for this training, with respect. Participant/Guardian is aware that they will be held liable for damage to property due to participant's own negligence.Participant/Guardian attests that the participant is in good physical condition and have no known or suspected medical conditions that would preclude vigorous physical activity.Participant/Guardian attests that the participant will not participate if they, nor any member(s) of my household, is diagnosed to be infected of COVID-19 virus within the last 5 days. Furthermore, participant will not train if they currently have a body temperature of 100+ degrees Fahrenheit, or symptoms of any other illness or contagious skin condition/infection until they are determined by a physician to be healthy or no longer have a fever or other symptoms of illness/infection.Participant/Guardian agree that the participant’s name, photograph or other representation for the purposes of promotion or publicity for Piasa Strategics, LLC (DBA Strategic BJJ) or the instructor(s) may be used.Participant/Guardian will ensure that participant maintains proper hygiene before coming to class, as well as the cleanliness of their training attire.Participant/Guardian will ensure that participant arrives at training sessions on time with appropriate attire and any PPE for the classes participant will attend. PPE includes, but is not limited to, Gi top/bottom/belt, rash guard, shorts, mouth guard, gloves, and shin guards.

This release shall also include any landlord or leaseholder of any training facility in which training is conducted.Participant is aware that their payment information will only be used per the membership level they sign up for, or seminar that the participant will be attending, or for any other transaction that is agreed to (verbally or in writing) prior to said transaction.As part of the consideration for participation, the participant/guardian acknowledges and assumes all risks and wishes to enroll in this course of instruction by Piasa Strategics, LLC. (DBA Strategic BJJ) or any party or parties leasing the facility.

By signing below I acknowledge that I have read the foregoing Liability Release Waiver and understand its contents; that I am at least eighteen (18) years old and fully competent to give my consent; That I have been sufficiently informed of the risks involved and give my voluntary consent in signing it as my own free act and deed; that I give my voluntary consent in signing this Liability Release Waiver as my own free act and deed with full intention to be bound by the same, and free from any inducement or representation. This waiver will remain effective indefinitely.
