Moonlight has enrolled in the NYSDMV LENS program (New York State Department of Motor Vehicles License Event Notification Service) to insure that our drivers maintain an acceptable driving record while employed by us.
Moonlight will enroll all employees in this system to obtain both baseline information and to monitor any change in an employee's driving record. These changes include, but are not limited to, reportable accidents, convictions, expirations, point and insurance reduction program completions, restorations, suspensions, and revocations.
PERMISSION: I hereby authorize Moonlight Horticultural Services to enroll me in New York State Deptartment of Motor Vehicles License Event Notification Service. I understand that this system allows for up to date information to be sent to Moonlight at all times during my employment. Furthermore, I understand that the information that Moonlight Horticultural Services receives from this site may affect my position and employment with Moonlight. (Unacceptable driving records, as determined by Moonlight, may result in demotion or termination.) I authorize Moonlight Horticultural Services to retain a copy of this information in mu employment file. I understand that upon written request, Moonlight will supply me with a copy of the NYS LENS infromation that it has in my file.
