Uniform Order Form- Credit Card Payments Only

Notes for use:

• One form must be filled for each family that attends our studios and wishes to order uniform. 
• Payment methods available are credit card, bank transfer, cash at the office or direct debit for payment plan customers (use seperate form).
• Uniform will not be ordered until payments are made.
• Orders will be placed with our supplier on a fortnightly basis confirmed via email once you place your order.
• Allow 3-4 weeks for your order to come in once it has been placed with our supplier.
• Please use the size charts to determine your child's size.  Bulk orders will not be placed in advance for the time being.
• Samples of each style will be available to view in studio but only a child's 10 and adults small sizes will be available.
• If an item does not fit appropriately, it can be returned in a new condition and suitable for resale and a new order can be placed. (excluding personalised items)
• Please try on all items at collection before you leave the studios.  Once you leave they will be unable to be returned.
• You will be notified by text when your order arrives and pick up is available in studio only.
• Personalisation is available for all Jacket, Jumper, T-Shirt or Tank orders for $15 each.  Please follow the direction at the bottom of the order form if you wish to add this to your garment.
