Tribe Hair & Company Hair Extension Agreement Form

I agree to the following:

I _________________________ agree that I am having hair extensions installed by Dakota Seger at Tribe Hair & Company on _____________. Client agrees not to have any other services, adjustments, or removals of extensions done by any other stylist who are not Bellami or JZ Styles certified.

Client agrees to book an appointment for 5-7 weeks for moveup/reinstall and color refresh. If client cannot, for any reason, commit to the upkeep of the service, the extensions will be removed by stylist and the hair is the property of the client. The hair may be reinstalled at a later date if the extension hair was not worn for more then 6 months and as long as stylist has agreed that the extensions are still in proper condition or new extensions will be required prior to install.

I agree that I will not hold Tribe Hair & Company liable in any way for any loss, damage, or injury suffered by me as a consequence of my use/or misuse of the hair extensions.

I will not bash Tribe Hair & Company on social media due to hair extension quality as they do not make or produce the hair in any way.
