1. CANCELLATIONS Cancellations should be made at least 24 hours in advance of a scheduled session. Sessions cancelled less than 24 hours in advance will be charged in full to the client.
2. LATE ARRIVALS Each session shall be 1 hour in length. Sessions will not be extended (unless time is available) due to the lateness of the client or due to interruptions caused by the client.
3. HEALTH AND SAFETY WAIVER The client acknowledges that participation in physical exercise and training activities involves a risk of injury. The client agrees to release and discharge [Your Name or Business Name] from any and all claims or causes of action (known or unknown) arising out of the negligence of [Your Name or Business Name], its employees, or agents, including any damages arising from personal injuries sustained while on the premises or under the supervision of [Your Name or Business Name]. The client agrees to voluntarily waive any claim that they may have against [Your Name or Business Name] for injury or damages that they may sustain as a result of participating in the program.
4. PAYMENT POLICY FOR MONTHLY PLANS All monthly plans must be paid for upfront at the beginning of each billing cycle. Failure to make payment will result in the suspension of services until payment is received.
5. ALL THE INFORMATION I HAVE GIVEN IS CORRECT All the information on this form is correct and to the best of my knowledge. I have sought and followed any necessary medical advice. I understand that all the information given will be kept confidential.
6. ONLINE TRAINING POLICIES
Clients participating in online training must ensure they have a safe space to exercise and are using equipment properly to avoid injury.
Clients are responsible for informing the trainer of any changes in their health status.
Regular check-ins and feedback are required to adjust the training program as needed for safety and effectiveness.
