Medical Release Form

Download the Medical Release Form. You may also pick up a form at any walk-in registration event or contact the registrar to give you a copy.

The Medical Release Form must be signed by your child’s primary care physician before she/he can participate in practice. This is an insurance requirement. No exceptions.

By registering your child with Rex Putnam Youth Cheer, you acknowledge that it is your responsibility to obtain the signature of your child’s primary care physician then provide your child’s coach with the completed Medical Release Form before your child can participate in any practice.

Fundraising Requirements*

Fundraising (or optional buy out) is a requirement for participant to particiapte in Rex Putnam Youth CheerCompleting registration you acknowledge that you are agreeing to participate in RPYC fundraising eventsI agree that one parent will contact the President or attend a walk-in registration event to receive or "buy-out" the required bags of pepperoni sticks and box of chocolates.
Uniform and Other Apparel

Registration cost does not include the cost of the uniform. Payment for the uniform is due on the uniform fitting day (June 11th, 2024)All RPYC participants are required to have current cheerleading uniform, cheerleading shoes, current hair bow, and current pom-poms as determined by the RPYC Board of DirectorsUniform will not be ordered unless paid in fullOther apparel may be required, practice or warm-up gear. Plan for additional expenses.
Competition

RPYC may have one or more competition squads. Competition is not required. If you would like more information please check the box. Note: there will be an additional fee for all competitions.
