By checking the following, you are stating that you understand and acknowledge these requirements for your child to participate in the Rex Putnam Youth Cheer (RPYC) program.
Fundraising Requirements*

Fundraising (or optional buy out) is a requirement for participant to particiapte in Rex Putnam Youth CheerCompleting registration you acknowledge that you are agreeing to participate in RPYC fundraising eventsI agree that one parent will contact the President or attend a walk-in registration event to receive or "buy-out" the required bags of pepperoni sticks and box of chocolates.
Uniform and Other Apparel

Registration cost does not include the cost of the uniform. Payment for the uniform is due on the uniform fitting day.All RPYC participants are required to have current cheerleading uniform, cheerleading shoes, current hair bow, and current pom-poms as determined by the RPYC Board of DirectorsUniform will not be ordered unless paid in fullOther apparel may be required, practice or warm-up gear. Plan for additional expenses.
Competition

RPYC may have one or more competition squads. Competition is not required. If you would like more information please check the box. Note: there will be an additional fee for all competitions.

RPYC TRANSPORTATION PERMISSION FORM

Please read carefully and fill out to give your athlete permission to ride share.
RPYC Transportation Permission Form/Waiver:

I give permission for my child to be transported by Rex Putnam Youth Cheer throughout the 2025-2026 season.

I understand that personal injury can and may occur to my child, and I hereby authorize Rex Putnam Youth Cheer, coaches and board members, to seek and consent to emergency medical attention for my child as needed; and I further agree to be liable for and to pay all costs incurred in connection with such medical attention.

I hereby release Rex Putnam Youth Cheer, its coaches, board members and volunteers, from any and all liability, claims, demands, causes of action and possible causes of action whatsoever arising out of or related to any loss, damage or injury (including death) that may be sustained by my child while participating in or traveling to and from the cheer events.

I give permission for my child to ride in any vehicle designated by Rex Putnam Youth Cheer, its employees and adult volunteers, while participating in and traveling to and from all cheer events throughout the 2025-2026 season.

I agree to accept full responsibility, financially or otherwise, for any damage my child may purposely do to the property of Rex Putnam Youth Cheer, properties visited on outings, other’s personal property, or vehicles used for transportation.

I understand that Rex Putnam Youth Cheer takes all precautions possible to ensure my child’s safety to and from all events.

Rex Putnam Youth Cheer PARENT & ATHLETE AGREEMENT:

As a parent and as an Athlete it is important to recognize the signs, symptoms, and behaviors of concussions. By signing this form you are stating that you understand the importance of recognizing and responding to the signs, symptoms, and behaviors of a concussion or head injury.

Parent Agreement: I have read the parent Concussion and head Injury Information and understand what a concussion is and how it may be caused. I also understand the common signs, symptoms, and behaviors. I agree that my child must be removed from practice/play if a concussion is suspected.
I understand that it is my responsibility to seek medical treatment if a suspected concussion is reported to me.
I understand that my child cannot return to practice/play until providing written clearance from an appropriate health care provider to his/her coach.
I understand the possible consequences of my child returning to practice/play too soon.

Medical Release Form

Please download and print the Medical Release Form at the bottom of the "Registration" tab on the website. You may also pick up a form at any walk-in registration event or contact the registrar or coach to give you a copy.

The Medical Release Form must be signed by your child’s primary care physician before she/he/they can participate in practice. This is an insurance requirement. No exceptions.

By registering your child with Rex Putnam Youth Cheer, you acknowledge that it is your responsibility to obtain the signature of your child’s primary care physician then provide your child’s coach with the completed Medical Release Form before your child can participate in any practice.

**The cost of the uniform, warmups and other incidentals are NOT included in the registration fee. Uniforms are roughly an additional $500 and due at the beginning of July.
