It is the responsibility of the applicant to ensure that three (3) references are sent to the Christian Church of Mid-America by May 30, at midnight CST. Please give your referents a copy of the reference form. Ask them to be specific in the matter of potential for ministry, character, academic aptitude and relational skills. In order for full consideration of your application references are needed from each of the following:
1. Pastor of the congregation of which you are a member or currently participating (unless spouse or family member, in which case ask another minister).
2. Dean, professor or counselor or academic institution.
3. Professional references who knows you well.
VIII. Transcripts
Please make arrangements for the Christian Church of Mid-America to receive official transcripts from your last completed semester in school by May 31, at midnight CST .
IX. Certification Statement
I certify that all of the information reported on this application is correct, to the best of my knowledge. I agree that if I am awarded a scholarship from the Christian Ministries Scholarship Fund, I will use the funds exclusively for tuition and/or books. I also agree that, if I receive a scholarship, I will submit to the Christian Church Mid-America Office confirmation from the school where I am currently enrolled. Also, after the fall term of this academic year I will provide proof of enrollment for the next semester to the CCMA Office. If I am unable to complete the course work or to apply the funds for my tuition and/or books, I will return the scholarship award, or the portion not used, to the Christian Ministries Scholarship Fund.

X. Evaluation
Your completed application packet will be considered with all other applicants. Each portion of the application is considered and assigned a numerical value. In practice, the committee might assign 20 points to each areas to have a potential score of 100. The logical five areas are 1) sections 1-4 that are the general profile, education, religious, affiliation and employment history; 2) financial need; 3) candidate’s statements or career goals; 4) quality of references and all three of th em being present; 5) quality of academic standing based on the transcript. It is very important for you to check with you referents to ensure the timely submission of their letters. It is your responsibility to make sure references respond by May 30, at midnight CST.
