The undersigned does hereby agree to indemnify and hold harmless CHVB Boosters Inc., the Grapevine-Colleyville Independent School District, and any associated Board of Trustees, agents, servants, employees, volunteers, coaches in both their individual and official capacity, and any other person, firm, or corporation bound to defend or pay judgements against it and/or them, or any of them, from and against any and all claims, demands or causes of action, including claims for contribution or indemnity, and the reasonable and necessary costs, including attorney's fees incurred in defense of any such claim, which any other persons, firm, or corporation has or may have arising out of/or resulting from child or individual participation in the CHVB ColleyVolley Grass Volleyball Tournament, or in any way connected thereto.

The undersign parent/guardian fully understands attendance is consent to be photographed, filmed and/or otherwise recorded. Attendance constitutes consent to such photography, filming and/or recording and to any use, in any and all media throughout the universe in perpetuity, of your appearance, voice and name for any purpose whatsoever in connection with the CHVB ColleyVolley Grass Volleyball Tournament.

The undersign parent/guardian warrant that they are the legal guardians of the child indicated in the PLAYER NAME fiels and that no other legal guardian has been appointed.

The undersigned warrant that he/she has read this agreement and fully understand it to be a release of all claims, known or unknown, present or future, that he/she has or may have against the party or parties released, arising out of or in any way connected with the matters described herein.
