Volunteers under the age of 18 years are required to provide parental/ guardian permission and consent to be assigned and participate as a volunteer for the Division of Parks & Recreation. All youth participants under the age of 14 years are required to be accompanied by an adult, unless assigned to a Volunteer program providing supervision. I certify that my child is fully capable of participating as a volunteer without compensation, and has my permission to be assigned and participate at blanks State Park(s) as a volunteer for the Division of Parks and Recreation.
By signing below, I understand and agree to the following:

I understand the risks involved in being a volunteer for the Division of Parks and Recreation and I understand that neither the State of Delaware, nor the Department of Natural Resources and Environmental Control, nor the Division of Parks & Recreation, nor any state employee or volunteer leader, will assume any financial liability for any injury or illness that might incur while my son/daughter is performing voluntary service for the Division of Parks and Recreation. Furthermore, I understand, and will abide by, the following Volunteer Policy:

Volunteers perform service without compensation and are not considered employees of the State of Delaware. The Division of Parks and Recreation does not provide Worker’s Compensation or any other insurance coverage for volunteers.

The Division accepts the service of all volunteers with the understanding that such service is at the sole discretion of the Division and is contingent upon successful completion at all levels of screening.

Photographs may be taken at park events and I give my permission to the Division of Parks and Recreation and Department of Natural Resources and Environmental Control to utilize photographs in future publications, promotional materials and exhibits. Photographs of the event may also be taken by media or sponsoring organization and may be used in news articles or news releases. I give my permission for such photographs to be taken and utilized in this manner.

Transportation to and from the project site may be provided on park work vehicles or boats run by qualified park employees or volunteer leaders.
