DECLARATION

By signing and submitting this form, I:
- Agree the information submitted is my own;
- Agree to be bound by the conditions of the GA Selection Policy (both Selection Policy Part A and Selection Policy Part B - Event Specific Criteria);
- Am an Australian citizen elgibile to represent Australia (Athletes ONLY);
- Acknowledge that selection to the event is on a self-funded basis and am prepared to fund this, should my nomination be successful;
- Agree to pay the $50 nomination fee, by the due date. This will be invoiced by Gymnastics Australia following the close of nominations (Athletes ONLY)
- I understand it is my responsibility to advise Gymnastics Australia in writing should I withdraw my nomination from the team.
- Understand the risks (including, but not limited to, financial and health risks) associated with travelling domestically during the ongoing COVID-19 pandemic.
