OMG YOUTH SPORTS COMPREHENSIVE WAIVER AND RELEASE OF LIABILITY
In consideration for my child being allowed to participate in sports activities organized by OMG Youth Sports and OMG Adventure Club, I, the undersigned parent or legal guardian, hereby agree to the following:
1. General Liability Waiver
I, the undersigned parent/guardian, understand that participation in the Power of Movement program involves physical activity, which may include movement, exercise, and use of adaptive equipment. I acknowledge that there is an inherent risk of injury in any physical activity, even when conducted with safety precautions and adaptations.
I voluntarily assume full responsibility for any risks of injury, illness, damage, or loss that may be sustained by my child or myself as a result of participation. I hereby release, discharge, and hold harmless OMG Youth Sports & Enrichment, its directors, officers, employees, contractors, agents, and volunteers from any and all claims, demands, costs, expenses, causes of action, or liability of any kind arising out of or in connection with my child’s participation in the program.
2. Medical Release Authorization
I authorize OMG Youth Sports & Enrichment staff or representatives to provide basic first aid to my child as deemed necessary. If my child needs emergency medical care and I cannot be reached, I give permission for the program staff to secure emergency medical treatment, including transportation by ambulance, hospitalization, anesthesia, surgery, or other medical care as recommended by a licensed medical provider.
I agree to be fully responsible for all costs associated with such medical care.
3. Assumption of Risk
I understand and acknowledge that despite careful and professional supervision, there are potential risks associated with participation in physical activity, including but not limited to falls, equipment-related injuries, sprains, and other physical or emotional injuries. I knowingly and freely assume all such risks, both known and unknown, and accept full responsibility for my child’s participation.
4. Media Release - Check box below
Yes- I grant permission for OMG Youth Sports & Enrichment to photograph, videotape, and/or record my child during program activities, and to use these images for marketing, social media, promotional, and educational purposes without compensation.
NO – I do not grant permission for my child’s image or likeness to be used.
5. Transportation Waiver (Only if applicable)
If transportation is provided by OMG Youth Sports & Enrichment staff, I grant permission for my child to be transported to and from program activities. I release OMG Youth Sports & Enrichment, its staff, and contractors from liability during transit, including but not limited to vehicle accidents, breakdowns, or delays, except in cases of gross negligence or intentional misconduct.
6. Behavior Agreement
I understand that my child is expected to follow all program rules and instructions to ensure safety and respect for all participants and staff. If my child engages in behavior that is unsafe, disruptive, or harmful, OMG Youth Sports & Enrichment reserves the right to remove my child from the session or program without refund.
7. Payment & Late Fee Agreement
I agree to pay all fees for the Power of Movement program as outlined in my invoice or service agreement. Payments are due by the agreed-upon date. I understand that late payments will incur a fee of $50 plus 1.5% monthly interest on the outstanding balance. Services may be suspended until payment is made in full.
8. Refund & Cancellation Policy
I understand that registration fees and deposits are non-refundable. Cancellations must be submitted in writing at least 14 days prior to the scheduled session start date to be eligible for a partial refund (less non-refundable deposit). No refunds will be issued for missed sessions, voluntary withdrawal, or removal from the program due to behavior violations.
Camp Credit Policy:
Camp credit may be issued only if requested at least 30 days prior to the camp start date.
Any requests made within 30 days of the camp start date are not eligible for refunds but may be considered for camp credit only, at the discretion of OMG Youth Sports.
After June 1, no refunds will be issued for any summer camp, and only camp credit may be offered, subject to OMG Youth Sports’ approval.
Transfers:
Camp transfers are not permitted unless explicitly approved in writing by OMG Youth Sports.
Unauthorized transfers are strictly prohibited.
Camp Credit Terms:
Camp credit is non-transferable, has no cash value, and must be used within 12 months of the originally scheduled program.
Credit may be applied only to future camps or eligible programs hosted by OMG Youth Sports.
Exceptions:
Exceptions will only be made in the case of medical emergencies with official documentation, and even then, refunds are not guaranteed and will be considered for camp credit only.
By registering for camp, all families agree to the above terms and conditions.

9. Illness & Injury Policy
I agree not to send my child to the program if they are ill, have a fever, or have a communicable disease. If my child becomes ill during a session, I will arrange for immediate pickup. I understand that if my child sustains an injury that prevents safe participation, medical clearance may be required before returning.
10. Acknowledgment of Policies
I have read and understand all program policies, including attendance, safety, communication, and behavior expectations. I agree to abide by these policies and to ensure my child does the same.
