Student Handbook Agreement

Review the Student Handbook (in English or Spanish) posted on this page. When you have finished, please proceed to fill out the form to acknowledge you have read and understand the policies and procedures.

Student Handbook Agreement

Failure to read the material contained in the student handbook does not excuse the student from observing the information and/or regulations stated. The Principal retains the right to amend the Handbook if necessary. If this occurs, parents/guardians will receive written notice.

Student Acknowledgment: I have read the Student handbook for the school year 2025-2026. I understand the rules and will cooperate with the school in this regard.

El hecho de no leer el material aquí contenido no exonera al estudiante de cumplir con la información y/o las normas establecidas. La Dirección se reserva el derecho de corregir el Manual, de ser necesario. En caso de llevarse a cabo dicha corrección, los padres/ apoderados serán notificados por escrito.

Aceptación por parte del Estudiante: He leído el Manual del estudiante para el año escolar 2025-2026. Comprendo las reglas y cooperaré con la escuela en este aspecto.

Additionally, I hereby consent to the use of my image, likeness, name, schoolwork, and/or work product, in whole or in part, in any and all media worldwide at any time now or in the future for purposes of advertising and promoting Cristo Rey Jesuit High School and/or the Cristo Rey Work Study Program (collectively, “Cristo Rey”) and approved Cristo Rey Partners and Sponsors without additional compensation. I release Cristo Rey from any liability or claims arising out of the use of my image, likeness, name, schoolwork, and/or work product, and I hereby waive any right that I may have to review or approve the form and use of any such materials.

Asimismo, por medio de la presente, autorizo el uso de mi imagen, apariencia, nombre, trabajo escolar, y/o producto de trabajo, en todo o en parte, en cualquier medio de comunicación mundial en la actualidad o en el futuro con el fin de promocionar a Cristo Rey Jesuit High School y/o al Programa de Capacitación Corporativa (colectivamente “Cristo Rey”) al igual que colegas y patrocinadores aprobados sin compensación adicional. Libero a Cristo Rey de cualquier responsabilidad o reclamo que surjan por el uso de mi imagen, apariencia, nombre, trabajo escolar, y/o producto de trabajo y por medio de la presente, renuncio a cualquier derecho que podría tener para examinar o aprobar la forma y el uso de tales materiales.

Parent Handbook Agreement

Parent/Guardian Acknowledgment: I am the legal parent or guardian of the above signed student. I have reviewed a copy of the Student Handbook. I understand the rules and will work with the school to support these policies.

Additionally, I hereby give my permission and grant all rights necessary on behalf of myself and my daughter/son/dependent to Cristo Rey Jesuit High School, Cristo Rey Work Study Program, and their respective agents and anyone authorized by them (collectively, “Cristo Rey”) for the following:

Aceptación por parte del Padre/Apoderado: Yo, el padre o apoderado del estudiante arriba firmante, he revisado el Manual del Estudiante. Comprendo las reglas y colaboraré con la escuela apoyando las mismas.

Asimismo, por medio de lo presente, autorizo y otorgo todos los derechos necesarios en nombre de mí mismo y de mi hija/hijo/menor
de edad a Cristo Rey Jesuit High School, al Programa de Capacitación Corporativa, y sus respectivos agentes y cualquier personaautorizada por ellos (colectivamente, “Cristo Rey”) a realizar lo siguiente:

On behalf of myself and my daughter/son/dependent, I hereby release Cristo Rey from any liability or claims arising out of the use of my daughter/son/dependent’s image, likeness, name, schoolwork, and/or work product, and I hereby waive any right that I or my daughter/son/dependent may have to review or approve the form and use of any such materials.

En nombre mío y de mi hija/hijo/menor de edad, por medio de la presente, libero a Cristo Rey de cualquier responsabilidad o reclamación que surjan a través del uso de la imagen, apariencia, nombre, trabajo escolar, y/o producto de trabajo de mi hija/hijo/menor de edad, y por medio de la presente renuncio a cualquier derecho que podamos tener para examinar o aprobar la forma y el uso de tales materiales.
