Val’s Appointment Request Form

Hi I’m Val, and I’m excited to work with you on your next tattoo project! Please provide details below about your desired tattoo and any reference images. I’m open to a variety of styles, so feel free to be creative with your ideas! I’m currently not offering cover-up tattoos.

Please note that the shop minimum is $100, and a deposit is required to secure your spot. The deposit will be applied toward the total cost of your tattoo and can be sent via Cash App or Venmo. Once you complete the form, I’ll reach out to you via email or messaging within 3-5 days. Please don’t hesitate to reach out if you have any questions. Thank you!
