I, as a parent or legal guardian, hereby give my permission for my child to participate in our kid's Nature Reconnection program, for survival training and primitive skills. I understand that this involves physical activity and presents the risk of injury. I am willing to permit my child to assume this risk and accept full responsibility for his/her participation in our kid's nature reconnection program and any potential risk involved with participation. If my child is injured while participating in class, he/she will notify the Instructor immediately. I have read and understand this form, the information it contains, and the program to which it pertains. I understand the program’s concept, structure, benefits, and risks. I have been given the opportunity to ask questions about this program. I understand that Nature Reconnection and Eleven Elevation assumes no responsibility for injury resulting from my child’s participation in this program. I hereby indemnify, hold harmless and release Eleven Elevation, Nature Reconnection, its instructors, charter schools, and property owners, from any and all loss, claims, injury, damage, or liability which might arise out of my child’s participation in this program.

During our classes, we take PHOTOS and VIDEOS to post. I understand that any images of my child may be used (without names or other identifying information) for Nature Reconnection/ Eleven Elevation brochures, website, and Facebook/Instagram page.
