I understand that I have two week course or four week course to access all the online didactic lectures. I have read handbook and had all my questions answered prior to signing this form. I know that the $150 registration fee is completely non-refundable and all course fees are non-refundable three (3) days after payment. I have retained a copy of this signed enrollment acknowledgment for my records.
