    Please fill out this questionnaire prior to your pet's stay with us AT LEAST 48 HOURS IN ADVANCE. Our check in times for boarding are between 11AM and 3PM and our check out times are between 8AM and 10AM. If your form is not completed at least 72 hours in advance we may not receive it in time for your reservation. IF YOU ARE USING YOUR PHONE TO COMPLETE THIS WE RECOMMEND TURNING YOUR PHONE HORIZONTALLY TO FULLY SEE THE QUESTIONS.

    If your pet is on any medications including supplements and multivitamins please fill out below. We ask that all medication be brought in the original container/packaging and not prepackaged into your dog's food.
  • Feeding Instructions

    We ask that clients bring the meals for their dog(s) pre-portioned in individual bags. If you have more than one dog coming for boarding please label which bag is for which dog. If their meals are different for each feeding please label what meal each bag is for. Example: 1 bag labeled "Maisy- Breakfast". PLEASE MAKE SURE TO PACK ENOUGH FOOD FOR YOUR PET'S ENTIRE STAY. Fetch recommends packing at least 1 extra day of food in case of emergencies.
  • If your dog uses a slow feeder bowl or elevated feeder at home we ask that you bring it in for their stay. Please select what you will be bringing.*

  • Sometimes dogs don't want to eat when they are first boarding. Fetch offers free of charge "toppers" that can be added to their meals to entice them to eat. We suggest allowing pumpkin and rice in case your dog has loose stool or an upset stomach while they are with us. We recommend authorizing AT LEAST 3 toppers.   • Additional Services

  • Fetch offers different things to customize your pet's stay with us. Please select below if you are interested in adding any of the following (these will be confirmed at the time of check-in, any additional fees will be added to their total collected at the end of the stay).
  • If you would like for your dog to have one of our spa services please select below. We offer these services on the day they are being picked up- please be aware that these are scheduled based on availability. We recommend calling in advance to reserve a spot if you would like to ensure they can receive a spa service. Prices will vary based on coat length, size and time it takes to complete service.

  • Payments & Cancellation Policies

  • Owner agrees to pay the rate for lodging in effect on the date the pet is checked into Fetch Pet Resort. All charges incurred by the owner shall be payable upon pick-up of the pet. The dates agreed upon at the time of check-in are required to be paid for. If an owner shortens the stay while the dog is in our care, the days originally booked will have to be paid for. If you cancel your reservation at least 14+ days ahead of your reservation start date, you will be eligible for a Fetch store credit equal to the full amount of your deposit. If you cancel your reservation within the 14 day window, your deposit will be forfeited and no store credit will be issued.

  • Deposit Policy

  • A 50% non-refundable deposit is required per animal to book a reservation. If the reservation is canceled, we require a 14+ day notice to receive a Fetch store credit equal to the amount of your deposit. Store credit can be used for any service except future deposits/gift cards. If the reservation was canceled in less than 14 days, the deposit will be forfeited.  Forfeiture of deposit will still occur if your pet is not up to date on the required vaccinations (Rabies, Distemper & Bordetella) in time for their reservation and it is cancelled less than 3 days in advance. 

  • Behavior Policy

  • If your animal becomes aggressive with our staff members during their stay, and it is not something that we can SAFELY work through, the animal(s) will need to be picked up by the owner(s) or a designated emergency contact. 

    We hope you understand that we do not want any animal to be in stress under our care, or to risk the health of our staff members; therefore, we will not continue to board or hold an animal that is exhibiting aggressive behavior toward our team.
