  • To be considered for admission, you'll need to submit one document with three key parts.

     

    What to Include:

    Statement of Interest (300-400 words): Tell us why you're drawn to the OSLEP program and how your chosen seminar aligns with your academic goals. Explain how participating will benefit your studies and future aspirations.

    Writing Sample (1500+ words): We want to see your academic writing skills in action. Submit a research paper (at least five pages) you wrote for a previous college course. Make sure it was solely your work, not a group project.

    Reflection on Your Writing (250+ words): This is your chance to delve deeper into your writing sample. Explain what makes you proud of this paper and how the research and writing process expanded your knowledge in the subject area.
     

    How to Submit:

    Combine all three parts into a single Word or PDF document. Make sure each section is clearly labeled for easy reading.

  • Photography Release

    I hereby grant permission to The Oklahoma Scholar-Leadership Enrichment Program (OSLEP) representatives, to take and use: photographs and/or digital images of me for use in news releases and/or educational materials. These materials might include printed or electronic publications, Web sites or other electronic communications. I further agree that my name and identity may be revealed in descriptive text or commentary in connection with the image(s). I authorize the use of these images without compensation to me. All prints and digital reproductions shall be the property of OSLEP.

  • Acknowledgement, Waiver, and General Release

     

    I will be participating in an OSLEP seminar, sponsored by the Oklahoma Scholar-Leadership Enrichment Program. 

    I understand that OSLEP seminars may carry inherent risks. I understand that factors beyond anyone’s control, including negligence, may affect my safety and well-being. In signing this Acknowledgement, Waiver, and General Release, I acknowledge that  participate willingly and assume all risks associated with my participation.

    If injured, I will rely solely on my own insurance or resources to cover any medical bills or other expenses or losses. I understand that no workers’ compensation or third-party insurance will be available to my family from OSLEP.

    On behalf of my family, I hereby release and promise not to sue OSLEP and its officers, directors, agents, representatives, employees, contractors, affiliates, successors, and assigns (collectively, “OSLEP”) from any and all liability for any claims or losses that I now have, ever had, or may ever have, related, in any way, to my activities in relation to an OSLEP seminar. Further, I agree to indemnify and hold harmless each member of OSLEP from any and all liabilities or claims made as a result of or in any way related to my participation in OSLEP, whether caused by negligence or otherwise. I intend that this Acknowledgement, Waiver, and General Release be construed broadly to the maximum extent permissible under applicable law.

    In the event of a medical emergency, I hereby give OSLEP my permission to secure treatment for me and, if necessary, to secure my transportation to the nearest suitable hospital, all at my expense.
