Field Trip/Permission to Treat
I give my consent for my child to participate in (a) all field trips not going beyond the boundaries of Pratt County and (b) all school sponsored athletic/extra curricular trips regardless of location.I further give my legal consent and authorize any representative of Skyline School to authorize emergency medical treatment, including any necessary surgery or hospitalization, for my above named child, for any injury or illness of an emergency nature he/she incurred while participating in the field trip or other activity noted above by any physician or dentist licensed in accordance with the provisions of the Kansas Healing Arts Act, L.S.A. 65-2801, and any hospital.I agree to pay and assume all responsibility for medical and hospital expenses and any emergency services incurred on behalf of my child.I acknowledge and agree that Skyline School is not responsible for any medical, hospital expenses and/or other charges that are incurred in the medical treatment or hospitalization of my child. Internet Publishing Consent and Release Agreement:Students who attend school in the Skyline Unified School District 438 are occasionally asked to be a part of school Web pages. In order to guarantee student privacy and ensure your agreement for your student to participate, the District asks that you and the student sign and return this form to the school for each of your children. The form referenced below indicates approval for the student's name, photograph, art, written work, voice, verbal statements or portraits (video or still) to appear in the school publications or school Web site in accordance with the Internet Consent Policy. Consent and Release AgreementI do hereby give Skyline Schools the right to use my name, photograph, art, written work, voice, verbal statements or portraits (video or still) to be reproduced on the Internet. This material will only be used for activities related to the Skyline Web site or school publications. I am the parent or the legal guardian of the above-named minor and hereby approve the foregoing and consent to the use of photograph, name, and published project pursuant to the terms mentioned above. PLEASE NOTE: Skyline Schools, USD438, does NOT provide medical insurance for students. It is the responsibility of the parent/guardian to provide such insurance for their child(ren). Skyline participates in a program that allows parents, on a voluntary basis, to purchase accident insurance from Student Assurance Services, Inc. Below is a document that provides parents/guardians with information about purchasing insurance from Student Assurance Services, Inc. Purchase of accident insurance is totally voluntary and the school district is not involved in any way in the transaction between the parent/guardian and the insurance company (Student Assurance Services, Inc.) *

I understand that access is designed for educational purposes. I also recognize that employees of the school or school system may not be able to restrict access to all controversial materials. I will not hold them responsible for materials my son or daughter acquires as a result of the use of the Internet from school facilities. I accept full responsibility for supervision if and when my student’s use of information systems is outside the school setting. I herby give my permission to USD 438 to permit my child to access and use the available information retrieval technologies.
