In consideration of the acceptance of this registration for Swing FOR The WIN ("Event") by For The Win, Inc. ("FTW"), the undersigned does hereby: (1) Release FTW and its committees, directors, volunteers, sponsors, partners (including The Golf Club at Indigo Run, the Old Oyster Factory and Frosty Frog Cafe) and agents from any and all liability arising out of or related to the Event; (2) Release all other participants and officials from any and all liability arising out of or related to the Event; (3) Warrant that the undersigned has been given a copy and personally read the rules and regulations related to the Event; (4) Give permission to use photos taken the day of the Event for publication and future promotion of FTW and the Event. *
