Our office does not accept insurance, Medicare and Medicaid and we do not file claims to the insurance companies, Medicare or Medicaid. Upon request to the office manager, Rikki, we can provide you with your Statement of Service and receipt if you would like to file yourself for out of network reimbursement, if your policy allows. This applies to all insurance companies and Medicaid with the exception of Medicare as Dr. Fox has a current opt-out contract with Medicare which does not allow for out of network reimbursement. We ask all patients that have Medicare coverage to review and sign the Physician/Beneficiary Private Contract Agreement. To stay within the Medicare opt-out contract guidelines, this agreement must be signed by both parties prior to the start of psychiatric services provided by Dr. Fox. If at any point in time you start recieving Medicare coverage while under Dr. Fox's care, you must notify our office and have submitted a signed Physician/Beneficiary Private Contract Agreement.
All fees are self-pay and are due at the time of each appointment. In order to keep billing costs down and to minimize paper usage, we no longer accept checks as a form of payment. We use a secure encrypted paperless billing system that includes having a card on file to pay for all appointments. We accept VISA, Mastercard, American Express, Discover and all Health Savings Account cards. All fees are posted to our website and in our office as well.
Per our cancellation policy that is posted on our website, patients are responsible for the full fee of all missed appointments and all late cancellations, without a 24 hour notice.
