By participating voluntarily in the services offered by Kimberly Bridgewater and Bridgewater Pilates, I agree I do so at my own risk.

I assume full responsibility for any injuries or damages which may occur using studio facilities and equipment, I do hereby knowingly and voluntarily forever waive, release, discharge, and hold harmless Kimberly Bridgewater and Bridgewater Pilates and all others acting on their behalf from any and all claims or liabilities for injuries, damages, loses, demands, and causes of action including, without limitation, negligence, arising out of or connected in any way to my participation in any activities, programs or services of Kimberly Bridgewater and Bridgewater Pilates and my use of any equipment or machinery provided by and/or connected with Kimberly Bridgewater and Bridgewater Pilates.

In consideration of being permitted to the use of facilities and equipment located at The Space and 5 Point Physical Therapy, I understand that Kimberly Bridgewater and Bridgewater Pilates shall not be liable for the loss or theft of, or damage to, the personal property of a guest or client.

I warrant, accept, and agree that I am physically sound and have no disability, impairment, or ailment preventing me from participating in active or passive exercise that would be detrimental to my health, safety, or physical condition if I do so engage.

I acknowledge that I am fully liable for all medical expenses in the event or result of injury, accident, and/or illness in participation with services offered by Kimberly Bridgewater and Bridgewater Pilates.

I represent that I have carefully read and understand this liability waiver and release.
