  • Client Consent: Waxing

    This form is required prior to any waxing service. If you are a returning client for waxing services, we kindly ask that you only fill out the check-in form if any of your information has changed since your last visit. If there have been no changes to your information, you do not need to fill out the forms. Simply let us know when you arrive for your appointment, and we will confirm your appointment details with you. Thank you for your cooperation in this matter.

  • IF YOU ARE USING ANY OF THE FOLLOWING MEDICATIONS, YOU CAN NOT BE WAXED TODAY: ACCUTANE, ADAPALENE, ISOTRETINOIN, RETIN-A, RENOVA, ALUSTRA, AVITA, TAZAROTENE, TRETINOIN, AVAGE OR DIFFERIN.

     

    YOU MAY EXPERIENCE SKIN SENSITIVITY/THINNING, WHICH CAN RESULT IN SKIN LIFTING, FROM THE FOLLOWING: SUNBURNED SKIN, RETINOL, CERTAIN MEDICAL CONDITIONS PREGNANCY, ANTIBIOTICS, OTHER MEDICATIONS NOT LISTED, MENSTRUATION

  • I understand the risks and dangers of waxing and agree to hold harmless Deluxe Wax Studio LLC and staff of all liability in relation to the service being provided. I agree if I begin use, or are currently using, any of the products listed in the above warning and do not inform Deluxe Wax Studio LLC prior to current or future treatments, I accept full responsibility for any adverse reactions.

    It is understood that waxing may cause some redness, bumps, soreness, and/or itching.

  • Policies

    Please read the policies below. This form is required before service. We express our gratitude for your cooperation in following our spa policies. Your compliance with our guidelines ensures a safe and comfortable environment for everyone and helps us provide the best possible experience. Thank you for choosing Deluxe Wax Studio.

  • Late Policy

    ● Please show up on time for your reservation.
    ● There is a 7-minute late grace period. You must be in the studio on the waxing table by the 7-minute mark or you will be considered late.
    ● If we are not able to accommodate you, the reservation will be cancelled and you may have to reschedule to a different time or date.


    Cancelation, Rescheduling, No-Show Policy

    ● If you need to reschedule or cancel your reservation, please notify within 6 hours of your scheduled appointment time. Please reach out via text, (630-492-0890) email, (deluxewaxstudio@gmail.com) or by going deluxewaxstudio.glossgenius.com


    ● Guests No-Showing, Canceling, or Rescheduling with less than 6 hours’ notice are required to pay 25% of the services booked. Paying the fee allows you to book services in the future. This fee is non-refundable and non-transferable to another appointment. You can Zelle the 25% fee to this email - deluxewaxstudio@gmail.com OR you can add a card on file to your GlossGenius account.


    Child Policy

    ● We welcome children who are well-behaved and able to sit still during your appointment. 
    ● Children must always be accompanied by an adult while in the salon suite.
    ● Please be mindful of other guests who may be sensitive to noise and keep children quiet and calm during the appointment. 
    ● Children are NOT permitted to touch or play with any equipment or products.
    There is limited space in the salon suite, only 1-2 extra guests are allowed. 
     

    Payment Policy

    ● Please bring exact change if you are paying in cash.
    ● We use GlossGenius to process payments, which are partners with Stripe - the leading payment processer. We accept Visa, Amex, Mastercard, Discover, JCB, UnionPay, Apple Pay and Google Pay.  
    ● Payment is to be made in full upon completion of waxing services.
    ● Please be advised that all sales are final on services and products. 

  • By signing this form you understand and agree to the policies above. 

  • Parent/Guardian Consent

    Only complete this page if the waxing service is being booked for a minor (under 18) At our waxing spa, we understand that younger clients may wish to receive waxing services. We do offer waxing services for clients under the age of 18, but only with the signed consent of a parent or legal guardian. This policy is in place to ensure the safety and well-being of our younger clients, and to comply with legal regulations. If you are under the age of 18 and wish to receive waxing services, please have your parent or legal guardian sign this form or it can be signed by the parent at the time of your appointment.

  • I, (parent/guardian) authorize Deluxe Wax Studio LLC to provide waxing services to (name of minor)

  • Facial and body waxing can be started whenever the minor and parent feel comfortable. Parent/guardian consent is required for any waxing for those who are under 18. The parent/guardian must accompany the minor for the first reservation, it is optional to be in the studio while service(s) are being performed.

  • Social Media Photo Release Form

    Photo consent to be featured on social media accounts. If you feel comfortable, please fill this out so we can create more content. (Optional)

  • Authorization, Release, and Consent

    We would like to request your consent to take before and after photos and of your waxing treatments at our spa. Before the treatment we would take a photo of the area being waxed, and after the service we would take another photo. We may also create short videos for use on our social media channels, such as Instagram Reels, to showcase the quality of our services. We assure you that all pictures or videos taken will be tasteful and respectful of your privacy. Any photos or videos depicting Bikini / Brazilian Waxing will be censored to protect your modesty. Your consent is entirely voluntary and can be withdrawn at any time. Please complete the form below to indicate your agreement.
