Emergency Information

Medical Insurance:

The camp provides basic non-prescription medication/products for the following list of common, minor ailments. These are all stocked at the camp and are used on an "as-needed" basis to manage illness or injury. Dosage for these medications will be as directed on the package or the nurse's discretion. 

Campers do not need to bring ANY of the following products for themselves.  Is your camper ALLERGIC to or respond negatively to any of these common over the counter medications? Please tell us if there's any reason for us to withhold them from your camper. 

Our camp nurse, Julie Lau, will be in charge of all medications during camp. She will be available to speak with any parent who may have questions.  

General Health Information

Fusion Camp understands that food allergies are no joking matter and while we want to be able to serve all the campers that attend; we are not able to cater to food preferences in addition to true allergies. Our camp nurse station has limited storage for food that needs isolation. We are not set up to prepare food, but rather store prepared perishables for campers to pick up and eat.

If your child has a specific food need, please speak with the Youth leaders and nurse Julie Lau. 

Chronic Medical:

Liabilty Release, Acknowledgement and Consent Liability Release: I hereby give my approval for my child’s participation in any and all activities prepared by Fusion Bible Camp and Return Ministries. In exchange for the acceptance of said child’s candidacy by Fusion Bible Camp and Return Ministries, I assume all risk and hazards incidental to the conduct of the activities, and release, absolve and hold harmless Fusion Bible Camp and Return Ministries and all its respective officers, agents, and representatives from any and all liability for injuries to said child arising out of traveling to, participating in, or returning from camp activities.

Photo & Video Release: I hereby grant Fusion Bible Camp and Return Ministries my permission to use the photographs or video that include my child for any legal use, including but not limited to: publicity, copyright purposes, illustrations, advertising and web content.

Medical Release and Authorization: As Parent and/or Guardian of the named camper, I hereby authorize the diagnosis and treatment by a qualified and licensed medical professional, of the minor child, in the event of a medical emergency, which in the opinion of the attending medical professional, requires immediate attention to prevent further endangerment of the minor’s life, physical disfigurement, physical impairment, or other undue pain, suffering or discomfort, if delayed. Permission is hereby granted to the attending physician to proceed with any medical or minor surgical treatment, x-ray examination and immunizations for the named camper. In the event of an emergency arising out of serious illness, the need for major surgery, or significant accidental injury, I understand that every attempt will be made by the attending physician to contact me in the most expeditious way possible. This authorization is granted only after a reasonable effort has been made to reach me. Permission is also granted to the Fusion Bible Camp and Return Ministries staff to provide the needed emergency treatment prior to the camper's admission to the medical facility. This release is authorized and executed of my own free will, with the sole purpose of authorizing medical treatment under emergency circumstances, for the protection of life and limb of the named minor child, in my absence.  

Camper agrees to the following: 1. I have read and agree to abide by all camper rules. Fusion Camp Rules (right-click and open in new tab) 2. I understand that upon arrival to Fusion Bible Camp the use of electronics and phones may be limited or restricted.  3. I give full permission to Fusion Bible camp to search anything I bring to camp (bags, clothing, car...etc.). 4. I understand that any violation of camp rules may result in the contact of parents or guarding with the possibility of being sent home and forfeiting my camp fee.

Confirmation BY SIGNING BELOW, I ACKNOWLEDGING I AM DELIVERING AN ELECTRONIC SIGNATURE THAT WILL HAVE THE SAME EFFECT AND BE AS EQUALLY AS BINDING AS AN ACTUAL SIGNATURE.
