Documents Required:

1. Birth certificates of applicant and their mother.

2. Passport and License of applicant.

3. Ketuba from parents' wedding.

4. Legal marriage certificate issued at parents' wedding.

5. Confirmation of Single Status for the applicant to be signed by two Jewish persons, neither related to each other nor to the applicant. See Section 2f to download.

6. In case of second marriage, Gett certificate or Death certificate.
