    Please have pictures or pdf's of your Camper's insurance card(s) and immunization records BEFORE starting this form and on the same device you are completing this form on. You will need to upload these documents while registering. Payment will be collected at the completion of this form with a credit card or through PayPal. If the class sign up deadline is missed, your daughter's classes will be chosen for her or she may be removed from enrollment, any money paid will be refunded, and we will move on to the wait list.***

  • Health History and Examination Form

    Please answer the following questions about each camper to provide camp health care personnel the camper's background so they may provide appropriate health care.  Campers are not allowed to keep medication in their dorms.  All medication will be handed out by the camp nurses.  Please provide complete information so that camp can be aware of your child's needs.  

    *** If your child has any sensitivies to light, sound, motion, etc. PLEASE list and explain in one of the boxes below.  Camp hosts dance parties, sing alongs, chants, cheers, etc. and it can be quite noisy a lot of the time.  We want every child to feel welcome and comfortable, so we need to know in order to accomplish a successful, fun experience for EVERY camper! ***

  • Prescription and Over the Counter Medications

    Please answer the questions below regarding any medications that your camper takes or is allowed to take.  No medications are allowed to stay in the cabins and will only be distributed by the camp nurse(s).  Please bring to camp a list of all prescription and over the counter medications that your camper takes, the medications in the original bottles/packaging with the name, dosage, and prescribing physician listed on the packaging.  Bring enough medication to last the entire camp stay.  Please answer each question completely and thoroughly.

    I hereby authorize and grant representatives with Lutheran Girl Pioneers and LGP International Camp to treat and give the medications authorized to my camper.  I further give permission to LGP representative(s) to treat and/or seek treatment for any injuries that my camper may incur while I am not present.

    As the parent or guardian of a camper in the Lutheran Girl Pioneers International Camp Program, I recognize and acknowledge that there are certain risks of physical injury, and I agree to assume full risk of any injuries, damages or losses, which I may sustain as a result of allowing my child to participate in with any and all activities connected or associated with this program.  I agree to waive and relinquish all claims against Marinette County and its officers, agents, and employees I may have as a result of allowing my child to participate in the program for any and all claims from injuries, damages, or losses, which I may have or may incur on account of my negligence during my child's participation.  I agree to idemnify, hold harmless, and defend Marinette County and LGP International Camp, thier officers, agents, volunteers, and employees from all claims resulting from injuries, damages, and losses sustained during and arising out of the activities of the program.  I have read, fully understand, and agree to the above Release and Hold Harmless Agreement.

  •  In accordance with the Freedom of Information and Protection of Privacy Act, Lutheran Girl Pioneer International Camp require consent to use personal information for the purposes of promotion of Lutheran Girl Pioneers on a Local, District, and National Level.

         I, the undersigned, do hereby consent and agree that Lutheran Girl Pioneers have the right to use photographs and/or videos of my child and to use these photographs and videos in any and all media, now or hereafter known; exclusively for the purposes of the Lutheran Girl Pioneers program.

         I further consent that my child's name and identity may be revealed therein or by descriptive text or commentary.

         I do hereby release to Lutheran Girl Pioneers all rights to exhibit this work in print for the express purpose of furthering the Lutheran Girl Pioneers program in the local, district, or national organizations only.  I understand that there will be no financial or other remuneration for recording my child, either for initial or subsequent transmission or playback.

         I also understand that Lutheran Girl Pioneers are not responsible for any expense or liability incurred as a result of my child's participation in this recording, including medical expenses due to any sickness or injury incurred as a result.

         I represent that I am the legal parent or guardian of this child and have read and understand the foregoing statement and am compentent to excute this agreement.

  • LGP NATIONAL CAMP PHONE POLICY EXPLAINED


    Cell phones have become an everyday part of our lives, but their presence can be a drawback to the summer camp experience.  At LGP International Camp, we are implementing a “No Cell Phone Policy”.  We want to create a safe, distraction-free environment for our campers. Of course, girls will be able to use our camp phone to reach parents any time, and parents can reach their daughters by calling the camp phone as well. For those campers who use their cell phones to take photos, there are many decent, digital cameras available now for $30 or less. We will continue to post photos and videos to our social media sites for parents and family members.


    However, we are discouraging personal devices at our camp for the following reasons:


    First, cell phones can be a major distraction. Instead of fully immersing themselves in the camp experience, campers with phones can be tempted to text their friends at home or play mobile games.  We have seen first-hand over the last few years, how campers only interact with their devices and not each other during break times.


    Secondly, cell phones limit children’s independence. Summer camp is a time to be away from parents and to test your limits in a new place away from home with the support of new friends and acquaintances, Instead of challenging themselves to make new friends or overcome new challenges, girls with phones tend to use them whenever faced with any new difficulty, instead of reaching out to our leaders and staff.  This limits the personal growth, independence, and strength that summer camp can produce.  


    Lastly, as all parents know, it is difficult to control what sites or games the girls are accessing on their phones.  It is even more difficult at camp.  


    These reasons are why we believe that a no phone policy is needed.  We are working on ideas to be able to send messages between campers and families.  A couple of our leaders had heard of some camps printing out messages to and from campers and families and delivering them similar to our daily mail delivery.  We will update you more on this idea as we get it worked out. 


    In summary, what we want most is the best camp experience possible for your girls!!
