PROOF OF COVID VACCINATION REQUIRED 

All attendees MUST show proof of covid full vaccination including one booster. Date of last vaccine must be at least two weeks prior to this event.  You may upload a photo of your vaccination card with this form or email the photo to Lynn@creeksidecanines.com.  Your registration will not be accepted without this.   NO EXCEPTIONS! Please read COVID rules below carefully before you register.  Thanks!

REFUND POLICY:  SHOULD CHANGING REGULATIONS REGARDING COVID-19 CAUSE THE CANCELLATION OF THIS EVENT, A FULL REFUND WILL BE GIVEN. A FULL REFUND WILL BE GIVEN TO ANYONE WHO TESTS POSITIVE FOR COVID-19 at any time prior to the event but you must email proof of positive test result. Full refunds for working spots and audit spots will be given for any reason up to May 15th.   From May 16th to June 4) refunds for working spots (minus a $20 service fee) will be given ONLY if the spot can be re-sold and all other groups are sold out by the time of the event. NEW!  After June 4th, no changes wil be accepted for any reason. Auditors - no refunds after May 15th.  No exceptions, sorry!

WAIT LIST  POLICY - Please read.  

Quite often the first two or three on the list make it in.  Due to high demand, all wait list spots are prepaid.  No one will be placed on the wait list without filling out this form and paying in advance.  Why?  Because changes happen quickly and often quite close to the actual date of the event. There simply isn't time for me to email people, wait for an answer, wait for payment or for them to decline and move on to the next person.   If you don't get in, of course you will receive a full refund.  You may also convert to an audit spot with the difference being refuncded. However, if you wish to withdraw from the list or choose not to take a working spot should it become available, the refund policy is as follows:  You have until May 15th to withdraw for a full refund.  May 16th - June 4th, if a spot becomes available for you and you choose not to take it, you will only receive a refund if there is another person the list who takes the spot minus a $20 service fee.  Once the workshops have been completed, anyone on the wait list who did not get in will receive a fast, full refund.  Again, you may also choose to convert to an audit spot and the difference will be refunded.

COVID RULES

I AGREE TO THE FOLLOWING COVID RULES:

1. Masks are optional but this is subject to change at any time by local regulations or policy by Quicksilver Canine.  If you choose NOT to wear one, please maintain social distance and respect the choices of others.  If local regulations should change between now and the time of the seminar, you are agreeing to abide by whatever they are even if masks are required at all times.  If you cannot do this, please do not participate in these workshops.

2. All attendees will either upload a photo of their vaccination card with this form, email a photo or show their original covid vaccination card prior to entering the building. The last vaccine must be at least two weeks prior to June 10, 2022.  You will not receive a refund if you forget to bring your card so uploading or emailing a photo to Lynn@creeksidecanines.com is highly recommended.

3. Annette will have a headset with a speaker so she can be heard by everyone without being close. Bring hand sanitizer. We will all pitch in to set bars at every jump height change. You are responsible for hand sanitizing after touching obstacles.  

Creekside Canines Release of Liability & SPECIAL COVID RULES & WAIVER

In consideration for being allowed to participate and/or observe in training classes, offered by

Creekside Canines, located at 9016 Larchwood Dr, Kernersville, NC, or at Quicksilver Canine, located at 5600 Clarence Rd, Julian, NC (hereinafter referred to as “the Premises”), the undersigned does hereby fully understand the following: 

Undersigned acknowledges that the Premises and the animals, guests, items and activities thereon, pose dangers to people and animals. Such dangers include, by way of example and not  limitation; animals that can bite, trip, knock down, and fight, indoor and outdoor surfaces that may be slippery, equipment that can trip, persons who cannot control their dogs, and activities involving dogs that can result in injury from bites or other causes.  Accordingly, Undersigned, on behalf of him/her, his/her  spouse and minor children, and anyone else whom Undersigned brings onto the Premises of the Owner or Occupier, (a) waives any and all legal duties on the part of the Owner or Occupier to keep the Premises free of dangers and defects, and to warn of any of the same, and (b) releases Owner or Occupier from and assumes the risk of injuries, losses, damages, costs and expenses by any means above described, and other injuries, losses, damages, costs and expenses of every possible cause and description unless inflicted intentionally or recklessly by the agents and employees of Owner or Occupier. 

1. I understand that this waiver means I give up my right to bring any claims including for personal injury, death, disease or property losses, or any other loss, including but not limited to claims of negligence and give up any claim I may have to seek damages, whether known or unknown, foreseen or unforeseen.

2. I further release and acquit Lynn Brodie, Annette Alfonso, Creekside Canines, Quicksilver Canine and all other persons for any liability for medical or veterinary expenses that may be the result of any accident that may occur to me, my child or my animal.

3. I expressly understand this to mean that I am relinquishing and releasing my right to sue others in either negligence of for strict liability based on either another person’s act or the actions of their animal.

4. I hereby declare that I am of legal age and competent to sign this agreement and that if not, that my parent or legal guardian is in complete understanding and concurrence with this agreement. I understand this agreement. I agree to be bound by it.

COVID WAIVER:

I have read and agree to abide by the COVID-19 safety policies stated above.
By participating in this event, you may be exposing yourself to and/or increasing your risk of contracting or
spreading COVID-19.
To the best of my knowledge I do not have COVID-19 at the time of attending the this event. I attest that I have not been in contact with, or exposed to, anyone with the that I take full responsibility for my own health and safety during this event.
I will follow the guidelines/requirements of Creekside Canines and Quicksilver Canine to reduce the exposure and/or spreading the virus.
I will fully cooperate with any City, County of State guidelines that have jurisdiction in the area of the trial. I
fully submit that Creekside Canines, Quicksilver Canine and Annette Alfonso, are in no way liable for any present or future COVID-19 exposure incurred at any time by any person, in attendance or not in attendance during or after this event.
I have read and understand that I hereby choose to accept the risk of contracting COVID-19 for myself
and/or any family member in order to attend this event. 
I hereby forever release and waive my right to bring suit against Creekside Canines, Quicksilver Canine and/or Annette Alfonso.
