Accident Waiver & Release of Liability - I understand that I will be working as a volunteer with the city of St. Ann Shelter/SAPA. I understand that I will have direct contact in my home as a foster parent and that contact carries inherent risks of injury and damage to me personally; I will obey any instructions given to me by official staff of the City of St. Ann and will carefully read all messages pertaining to each animal with which i have contact. I acknowledge that my presence in my home as foster parent carry the potential for accidental injury and/or property loss. The risks include, but are not limited to, those caused by the condition of the facility, injury or illness as the result of general or direct contact with animals, the actions of other people and/or vehicles. I hereby assume all risks of acting as a volunteer. I represent that the information that I have provided on this form is the truth to the best of my knowledge and belief.
