I hereby grant permission to The Madison Playgroup to use my child's image (photographs and/or video) for The Madison Playgroup's nonprofit purposes. This includes use in print, in online spaces (such as The Madison Playgroup website and social media accounts), and all other forms of media.
I hereby waive any right to inspect or approve the finished photographs or electronic matter that may be used in conjunction with them now or in the future, whether that use is known to me or unknown. I also release The Madison Playgroup, its agents, and employees from all claims, demands, and liabilities in connection with the rights granted above
