POLICY
My time and resources are valuable. I often travel significant distances to provide services. In order to maintain a smooth scheduling process and ensure a positive experience for all, I kindly ask that both first-time and regular clients adhere to the following policy:
Deposit Fee & Payment:
All appointments are confirmed at least 24 hours in advance, or your slot will be offered to another client.
To secure your appointment, a $20 non-refundable deposit fee is required at the time of booking. This fee will be applied towards your session cost and ensures that your desired time slot is reserved for you. You can conveniently submit the booking fee via Zelle, CashApp, or Venmo using the link in my Instagram bio.
The remaining session balance is due at the time of your appointment, before the massage session begins. Tips are not required but of-course always appreciated. I accept cash, debit, and or credit. A 5% discount is available for clients paying in cash.
Cancellations/Rescheduling:
Clients are required to provide at least 24 hours' notice for any cancellations or rescheduling requests. If you cancel within this time frame, you can use the $20 booking fee towards rebooking a session within 30 days of the original appointment date, subject to availability. If the cancellation is made less than 24 hours in advance or in case of a no-show, the booking fee will be forfeited.
In the event that I needs to cancel or reschedule the treatment session, clients will be contacted 24-hrs in advance (unless there is an immediate emergency) and any payments will either be refunded in their original form or credited to their account for future use.
No Call-No Show:
To avoid disruptions, I have a strict no-show policy. Clients who miss their scheduled appointment without providing 24-hour notice may have their future bookings declined, unless there are extenuating circumstances.
Late Arrival:
I understand that unforeseen circumstances may arise; however, to ensure fairness and respect for all clients, it's important that everyone is available and ready at the agreed-upon time and location for their appointments. A 15-minute grace period is provided for "late arrivals".
If a client is unavailable at the specified time, please note that the session will still end at the originally scheduled time. Extending the session may not be possible to avoid delays or disruptions to other clients' appointments.
In the event that I am running late due to unforeseen circumstances (i.e., traffic), I will contact the client by phone. Rest assured that no time will be deducted from the scheduled treatment session in such instances.
Space Requirements:
Clients are responsible for providing a clean, private, and quiet space with enough room for the massage table and therapist to move around. The space should be free of pets and other distractions, with adequate lighting, temperature control, and access to water.
Mobile Procedures:
In order to ensure a seamless and well-prepared treatment session, I may arrive 10-15 minutes before the scheduled appointment time. This extra time will be used for setting up the treatment area, reviewing your intake form, discussing any updates to your health information or treatment goals, and giving me a chance to use the restroom. Clients do not need to be present during this setup period.
Clients are kindly asked to provide two clean, flat sheets for the massage table, a fresh pillowcase, and a hand towel for personal use. Additionally, please ensure access to an electrical outlet for any necessary equipment.
Health:
Clients are encouraged to inform me of any changes in their health status prior to each session.
For the health and wellbeing of all my clients and myself, it is imperative that clients refrain from receiving treatment when experiencing acute illness (i.e., cold, cough, flu, fever, stomach virus, fungal infection, contagious skin rash, etc.). If a client displays symptoms of any illness that contraindicates treatment, the session will be immediately suspended, and the client will be informed. The session will then be rescheduled for a time when it is safe to receive treatment. I will assist you with rescheduling.
Code of Ethics & Professional Conduct:
I am committed to creating an inclusive and welcoming environment for all clients, regardless of their race/ethnicity, gender or sexual orientation, and physical or mental ability.
Mutual respect and professionalism are essential in all interactions to ensure a comfortable and positive experience for everyone.
Clients will always be modestly draped with a full sheet. Only the area being massaged will be undraped. Under no circumstances will the breast and genital areas be massaged. The hip and gluteal areas will only be massaged with explicit client permission and can be worked through the draping if requested.
Inappropriate behavior, including suggestive comments, sexual innuendos, or attempts to intimidate the practitioner or sexualize the treatment session, will not be tolerated. Such actions will result in the immediate termination of the session and the therapeutic relationship.
Communication:
Open and prompt communication is vital in ensuring the best possible treatment experience. I encourage you to share any concerns, preferences, or feedback with me at any point during our work together. Your input is valued and helps me tailor my approach to best meet your needs. CLIENT AGREEMENT & LIABILITY RELEASE FORM
Client information are confidential and will not be discussed with anyone outside of the treatment session. Written authorization is required to release any information.
- I understand that massage or bodywork may be contraindicated for certain medical conditions or symptoms. A referral from my physician or licensed health care provider may be necessary prior to service being provided.
- I understand the purpose of massage or bodywork is to promote general wellness, reduce stress, encourage relaxation, and relieve of muscular tension. If I experience any pain or discomfort during my appointment, I agree to inform the therapist so that adjustments can be made.
- I acknowledge that massage or bodywork is not a substitute for medical care, and understand the importance of consulting a physician for any mental or physical conditions I may have.
- I understand that while every effort is made to ensure client safety, massage therapy can involve risks and potential side effects, such as short-term soreness, superficial bruising, allergic reactions to massage oils or lotions, or exacerbation of undiscovered injury.
- I am aware that massage therapists and bodyworkers cannot perform skeletal adjustments, diagnose, prescribe, or provide medical advice for illnesses or disease.
- Either I or the therapist, Natasha Levy, may end the session at any time for any reason. Any inappropriate remarks or actions will result in the immediate termination of the session and therapeutic relationship.
- I agree to maintain open communication about my health status and and agree to inform my therapist of any new diagnoses, medications, or changes in my health prior to each session.
- My consent is informed and voluntary, and I understand that I may withdraw my consent at any time except for actions already taken.
- I release the Natasha Levy and LVYN Wellness from all liability for any harm that may unintentionally result from this treatment.
