    My child, listed below in the applicant section of this form, has my permission to participate as a member of the 2026-2027 All Star cheer season with BOMBSHELLS YOUTH CHEER AND DANCE ASSOCIATION. I understand that he/she must abide by the rules and regulations set forth by the PROGRAM CHEER DIRECTOR and THE COACHING STAFF, and be present for all practices. I have read the rules and regulations and understand that the violation of any of these rules may lead to temporary or permanent suspension from the program. I understand that all forms attached must be completed before my child will be added to the roster, my commitment fee of $300 (RETURNER), MUST be paid by May 15th or $300 (NEW ATHLETE) by May 1, 2026 and I understand these fees are NON-REFUNDABLE. (There will be a fundraiser provided to you in April to help with these cost if interested) I understand that my child will not be allowed to participate in any activities until this is completed. I understand that my child must attend all practices either virtual or in person (unless excused by the director or coaching staff). I understand that once I sign the final contracts that will be provided to me, I am responsible for all costs involved as stated in the contract & packet. I also understand that BOMBSHELLS YOUTH CHEER & DANCE has a NO refund policy which includes initial registration and any additional fees associated with the program regardless of the circumstances. I understand also, that if my child or myself is removed from the team due to disrespectful behavior there will be NO refund given. I understand that myself or my child may be terminated from the program involuntarily at any time for violating the contact or Season 6 rules and parent handbook or program contract.

    I understand by the very nature of the activity, cheerleading and gymnastics carry a risk of physical injury. No matter how careful the participant and coach are, how many spotters are used, or what landing surface is used, the risk cannot be eliminated. The risk of injury includes minor injuries such as muscle pulls, dislocation, and broken bones. The risk also includes catastrophic injuries such as permanent paralysis or even death from landing or falls on the back, neck, or head. I understand these risks and will NOT hold BOMBSHELLS YOUTH CHEER & DANCE ASSOCIATION LIABLE or any of its personnel responsible in the case of accident or injury at any time. I herby grant permission to allow my child to participate in cheer and dance activities along with team bonding activities sponsored by the Bombshells Youth Cheer and Dance Association. Furthermore, I hereby release the BOMBSHELLS YOUTH CHEER & DANCE, it's officials, agents, coaches, and trainers, from any legal or financial responsibility resulting from actions of, injury to my child engaged in any activities (i.e. games, practices, competitions, and team events). It is my understanding that every registered participant will be covered by their own personal accident insurance.

  • Do you understand that if your child is chosen to participate on a crossover team, athlete will be responsible for ALL crossover fees. A “Crossover” athlete is an athlete who compete on 2 teams*
  • Are you interested in your child be chosen to participate as a Flyer? If so, you agree that it is mandatory to invest in our flyer program? This includes all flyer classes and financial commitment.*
  • Do you understand that your athlete may be placed on a PREP or ELITE Allstar team if chosen? This also requires a FULL 12 month commitment which does require attending some out of state travel, attending and paying for mandatory camps/clinics and attending additional classes/practices if need. You also understand there will be additional cost associated with anything outside of monthly tuition.*

  • I understand that registering for this program, I am responsible for an 12 month financial monthly commitment for all novice, prep, or elite teams and an 8 month financial monthly commitment for all half season athletes. Financial monthly commitment includes but not limited to: Monthly tuition, competition uniform fee, warm up fee, practice wear, competition shoes, competition socks, competition bow, competition bag, team jacket, choreography fee, music fee, competition fees, team dues, clinics, booster fee and separate Nationals registration fees (If we get a bid)*
  • I understand that my athlete’s practice wear is NOT included in the commitment fee and this is a separate fee of $150 due by June 15th. Athlete will receive TWO practice sets once paid*
  • I understand there is a NON-REFUNDABLE $300 commitment fee due per athlete. This fee secures your spot in the program. If I do not pay this fee, I understand my athlete will not be allowed to be placed on a team until this is paid.*
  • I understand that my athlete must attend evaluations on the date/time specified by BSE at the gym to be considered for a team. Date/Time slots will be given once registered? I understand if I miss, I will be responsible for paying a $20 fee for an alternate date*
  • I understand that if I commit to this program, I am committing to a full 1 year commitment and even if I quit or am involuntarily dismissed form the program, I am still responsible for tuition each month until after May 1, 2027. I understand that there are also no refunds. I also understand that uniform, apparel and ALL items purchased belongs to BSE and if I quit or am removed from the program involuntarily, these items will not be returned to me.*
