Grant Conditions and Organization Requirements
Below are the conditions of the grant. By clicking each box you are acknowledging the requirements.*

I represent an organization working with students in an economically disadvantaged environment and/or other funding is not available.I understand that this grant will provide the Habitudes program, not compensation to be used for salaries, marketing, administrative costs, or loans. There will be no cash disbursements.I will complete, sign, and return the contract upon application approval.I will complete semi annual reports to be sent back to Growing Leaders that include pictures, video, and testimonies of the Habitudes® program twice per academic year. I will also administer pre and post surveys to students and facilitators in the Habitudes® programI understand that Growing Leaders reserves the right to withhold and/or cease access to resources if the above requirements are not met or if copyrighted materials are utilized outside of permission directly from Growing Leaders.I agree to report both returning students and new students to the program with each semi-annual report.
