In consideration of the Canadian Society of Exploration Geophysicists (“CSEG”) allowing me to participate in the CSEG T-WAVE TOURNEY, I waive any and all claims that I have or may have in the future against, and release from all liability and agree not to sue, the CSEG, any of their directors, officers, employees, servants, agents, representatives, volunteers (collectively the “Personnel”), as well as other participants, sponsoring agencies, sponsors, advertisers, and if applicable, owners and leasers of premises used for the CSEG T-WAVE TOURNEY for any personal injury, death, property damage or other loss that I sustain during or as a result of the CSEG T-WAVE TOURNEY due to any cause whatsoever on the part of anyone or more of CSEG, the Personnel or others, including:

Negligence
(includes failure to use such care as a reasonable, prudent and careful person would use under similar conditions)

Breech of any other duty imposed by law
, including any duty imposed by occupiers liability or other legislation

Breach of any contract
-
Mistakes or errors in judgment
of any kind
I am aware of the risks inherent in the CSEG T-WAVE TOURNEY golf tournament. These risks include, but are not limited to:

Personal Fitness Level
– I am aware of the risks of possible injury resulting from physical exercise. I hereby certify that I know of no medical problems that would increase my risk of illness or injury as a result of participation in this activity. Injury can result from:

Exerting and stretching various muscle groups;
-
Physical exertion, rapid movements and uneven folding.

Golf Equipment or Other Players
– I am aware that by participating in the CSEG T-WAVE TOURNEY Golf Tournament there is a possibility of bodily injury, partial or total disability, paralysis and/or death which may arise from being struck with or colliding with:

golf balls;
-
golf clubs;
-
golf carts;
-
other players.
-
Weather
– I am aware of the risks of possible injury resulting from variable weather conditions:
-
lightning strikes;
-
slippery conditions.
-
Ground Hazards
– I am aware of the risk arising from the conditions and use of a public golf course including the possibility of injury from:
-
surface imperfections;
-
sprinkler heads;
-
water, sand and other hazards;
-
driving or riding in a motor vehicle.

Due to the social aspect of this tournament I realize that I'm responsible for conducting myself in a professional and appropriate manner:
I am aware of the damages and hazards of:

eating food prepared by the tournament venue;
-
consuming alcoholic beverages.
I assume and accept all the risks and dangers of the CSEG T-WAVE TOURNEY and the possibility of personal injury, death, property damage or other loss as a result. I willingly accept these risks and agree to the terms of this CSEG T-WAVE TOURNEY Waiver, even if the CSEG or any of the Personnel are found in law to be negligent or in breach of a duty of care or any other obligation to me in their conduct of any CSEG T-WAVE TOURNEY.
In signing this CSEG T-WAVE TOURNEY Waiver, I am not relying on any oral, written or visual statements of any kind, including promotional statements made by CSEG in its brochures or by any of the Personnel to induce me into entering the CSEG T-WAVE TOURNEY.
I confirm that I am the full age of Eighteen (18) years, that I have had sufficient time to read and understand what I am agreeing to in this CSEG T-WAVE TOURNEY Waiver before signing it and that will be binding upon my heirs, next of kin, executors, administrators and successors.
I agree that the CSEG T-WAVE TOURNEY Waiver and the agreement it contains will be governed in all respects by and interpreted in accordance with the laws of the Province of Alberta and I hereby irrevocably attorn to the jurisdiction of the courts of the Province of Alberta.
