Elevate Three Basketball Training LLC Liability Waiver THIS IS A RELEASE OF LIABILITY, READ BEFORE SIGNING.

This agreement releases ElevateTHREE Basketball Training LLC, Skill Lab LLC, & ChikambaONE Media Group LLC, Indy Holdings (landlord), the City of Indianapolis from all liability relating to injuries that may occur on-site and/or during one of the basketball skills training sessions.

By signing this agreement, I agree to hold ElevateTHREE Basketball Training, LLC, Skill Lab LLC, & or ChikambaONE Media Group LLC, Indy Holdings (landlord), the City of Indianapolis entirely FREE from any liability, including financial responsibility for injuries, regardless of whether injuries are caused by negligence.

I also acknowledge the risks involved in the basketball training activity. These include but are not limited to injury, psychological, emotional, spiritual, and physical distress, and/or death. I swear that I am participating voluntarily and that all risks have been made clear to me.

Additionally, I do NOT have any conditions that will increase my likelihood of experiencing injuries while engaging in this activity. I also agree to reimburse ElevateTHREE Basketball Training, LLC, Skill Lab LLC, & or ChikambaONE Media Group LLC, & Indy Holdings (landlord) for damages I inflict on the property or any material goods owned by ElevateTHREE Basketball Training, LLC, Skill Lab LLC, & or ChikambaONE Media Group LLC, Indy Holdings (landlord). Training intentionally or unintentionally during the course of the event, at all cost deemed appropriate by ElevateTHREE Basketball Training, LLC, & ChikambaONE Media Group LLC, owners or staff.

I also acknowledge that ElevateTHREE Basketball Training, LLC, & ChikambaONE Media Group LLC, staff reserves the right to refuse me access or service if I engage in a manner that endangers others or the property for any reason, and at any time during training sessions, without refunding any portion of the cost of services.

By signing below, I forfeit all right to bring a suit against ElevateTHREE Basketball Training, LLC, Skill Lab LLC, & or ChikambaONE Media Group LLC, & Indy Holdings, (landlord) and its affiliates including the landlord of the property for any reason. I will also make every effort to obey safety precautions as listed in writing and as explained to me verbally. I will ask for clarification when needed.

For Parents/Guardians of a participant of minor age (under 18). This is to certify that I, as guardian with legal responsibility for this participant, do consent and agree to his/her release as provided above of all the Releases, and, for myself, my heirs, assigns, and next of kin, I release and agree to indemnify and hold harmless Skill Lab LLC, Indy Holdings (landlord) ChikambaONE Media Group LLC doing business as ElevateTHREE Basketball Training and it’s owners, employees, representatives, and agents from any and all liability incidents to my minor child’s involvement or participation in the activity as provided above. EVEN IF ARISING FROM THE NEGLIGENCE OF ChikambaONE Media Group LLC doing business as ElevateTHREE Basketball Training to the fullest extent permitted by law.
