The purpose of this survey and later interview is part of an Otago Polytechnic Master’s degree course that is researching school leaving aged student views on career pathways and the decisions and influences that help decide their planned career.

About The researcher: This research was derived from a skill shortage in Phil Meek's engineering business. Phil has engaged the Otago Polytechnic master’s degree programme together with research data to find strategies for his industry to better attract school leavers into this profession. www.mmautopak.com.au

I agree to participate in a research project led by Phillip Meek as part of his Master of Professional Practice degree programme. The purpose of this document is to specify the terms of my participation in this project through being interviewed.

1.        I have been given sufficient information about this research project. The purpose of my participation as an interviewee in this project has been explained to me and is clear

2.       My participation as an interviewee in this project is voluntary. 

3.       Participation involves being interviewed by Researcher Phil Meek as part of his Otago polytechnic master’s degree course. I allow the researcher to take written notes. I also may allow a recording of the interview for data collection only. It is clear to me that in the case I do not want to be taped I am at any point in time fully entitled to withdraw from the interview

4.       I don't have to answer any of the questions. If I feel uncomfortable in any way during the interview session, I have the right to withdraw from the interview.

5.       I have been given a guarantee that, the researcher will not identify me by name in any reports using information obtained from this interview and that my confidentiality as a participant in this study will remain secure.

6.       I have been given the guarantee that this research project has been reviewed and approved by the Otago Polytechnic Ethics committee. For research problems or any other questions regarding the research project, the researcher may be contacted through your schools’ careers advisor.

7.       I have read and understood the points and statements of this form. I have had all my questions answered to my satisfaction, and voluntarily agree to participate in this study.

8.       I will receive a copy of this consent form co-signed by the researcher and my guardian.
