What is the address of your usual residence?

Please provide the physical address (street number and name not post office box) where you usually reside rather than any temporary address at which you reside for training, work or other purposes before returning to your home.

If you are from a rural area use the address from your state or territory’s ‘rural property addressing’ or ‘numbering’ system as your residential street address.

Building/property name is the official place name or common usage name for an address site, including the name of a building, homestead, building complex, agricultural property, park or unbounded address site.

Who we should contact in case of emergency?

This is the legal person for Imagine Education Australia to contact in the event of an emergency. This person must be legally responsible for your welfare, i.e. a family member.

• I am aware of the material and equipment that I must have/provide to complete this qualification.
• Available payment options have been discussed and I am aware of the student contribution fee which is charged at $5.00 per unit (non-concession) and $3.00 per unit (concession). Training will not commence until the student contribution fee has been paid in full.
• I am aware that in the event that Imagine Education Australia cancels a course, I will be offered either equal course, equal qualification or a refund.
• I am aware that the full Refund Policy is located at http://imagineeducation.edu.au/
• I am aware that if under 18 years of age I must seek permission from a Parent/Guardian should I be required to leave the premises within class hours. Parent/Guardian must make direct contact with our head office prior to class commencement.
• Your course duration is outlined in the Confirmation of Studies and Training Plan. To facilitate a postive and beneficial learning experience for our committed students, regular attendance to training sessions is required. We reserve the right to cancel a student that is not performing or not delivering agree learning outcomes on time. The applies to students not attending scheduled training sessions or repeatedly cancelling appointments with trainers.
• I am aware of the mandatory 240 hours work placements required for CHCSS00090 - Supporting Children and Families with Complex Needs.
• I am aware that I must be an Existing Worker in Early Childhood Education and Care and already hold a Certificate III or higher level qualification.

• By signing below, I acknowledge that I have read and agree to the information provided.
• I understand that information contained in this form may be provided to State and Commonwealth agencies and research organisations and I consent to that occurring.
• I agree to abide by Imagine Education Australia Australia policies as outlined in the Student Handbook.
• I confirm that all information contained in this application is accurate at the time of submitting.
• I agree for my contact details to be added to an Imagine Education Australia mailing list to receive electronic communication from time to time. I will notify Imagine Education Australia if I do not wish to receive communication.
• I agree for Imagine Education Australia to video and/or take photos for quality training purposes and marketing.
• If you are under 18 years of age, a parent or legal guardian must sign this form.
• I agree for my academic progress and results to be shared with my Job Service Agency, Centrelink, School or Employer (whichever applies).
• I have been informed by the Imagine Education Australia Representative that upon completion of a certificate IV level or higher qualification I will no longer be eligible for a government subsidised training place under the Higher Level Skills program.
• I authorise Imagine Education Australia to search and apply for a USI on my behalf if I have not obtained a USI.

By submitting this application, you are not guaranteed a place within your chosen course until Imagine Education Australia has received and reviewed this application form and you have been provided with confirmation of your acceptance. Whilst every effort is made to ensure that a scheduled course does commence, Imagine Education Australia reserves the right to cancel, postpone, or otherwise modify any courses offered, locations delivered at, and delivery times.
