  • SWMS General Instructions:

    1. Write down in the (Job/Task column) a description of the job process or action that is being performed.

    2. Identify and write down the potential hazards (Potential Hazards column) associated with the job process or action.

    3. Use the Risk Matrix above to calculate the potential risk level for each hazard and enter the number (in the Risk Level column

    4. Risk Control Applied - select the most appropriate method and control measure to control the risk.

    5. Calculate and enter a post-risk level for each hazard, to make sure it is As Low As Reasonably Practicable (ALARP).

    6. Write down the person who is responsible for implementing the risk controls (in the Person Responsible column).

    7. The Supervisor is to monitor the risk controls daily (or as required) and review SWMS weekly (or as required) in consultation with relevant workers, to make sure the risk control measures remain effective in eliminating hazards and risk, or minimising risk to as low as reasonably practicable (ALARP).

    8. The Supervisor must hold a safety meeting (pre-start or toolbox) to induct workers into the revised sections of the SWMS.

  • 1. After completing a site-specific risk assessment ensure that all new hazards identified are recorded on the register below.
    2. Make sure that you match up the new hazard with the relevant SWMS Step number and task that the hazard is associated with.
    3. Write down the ‘Step number’ in the first column below so that workers can understand and follow the revised SWMS and new risk controls easily.
    4. The supervisor must complete the SWMS review register at the bottom of the last page in the SWMS, then initial and date.
