In the event, that the parent or guardian cannot be reached to make arrangements for emergency medical care, I authorized the teachers or the director of Little Dreamers give consent for any necessary emergency medical care for my child.
If any of the above information changes, I will notify Little Dreams Learning Center immediately in writing so that this form may be updated. Failure to do so may result in my child being unable to continue being enrolled at the Little Dreamer's Learning Center
Parents Responsibility

Parents/ guardian are required to produce a copy of their income tx returns once ayear and check stub once every three months (Jan, April, August and December)

Payments must be made prior to the new child beginning at Litte Dreamer's Learning Center each week for the full week WITH NO EXCEPTIONS for sickness or holidays. If payments are to be paid twice monthly or monthly. IT REWUIRES PRIOR TO APPROVAL BY THE DIRECTOR. A letter regarding past due balance will be given to the parent and if fees are not paid child will be dismessed.

The parents agrees to notify the L.D.L.C of any changes of the address, place of employment, change of salary, telephone and martial status immediately. Failure to do so may result in the child being dropped from the roll.

The parents must give two weeks notice when the child will be withdrawn from the center. All children must be clean both in body and in clothing when brought to the center. Please take the time to clean your child's face and hands if they have been eating during the ride to daycare.

Parents or guardians, hereby waive, discharge and release the L.D.L.C from all claims, demainds, liabilities, suits and causes of action, preset of future, against it in any manner claimed for any injury or injuries of whatsoever nature received by child/children of parent or guardian while in custody of the Little Dreamer's Learning Center

The parent must sign in her/his child when arriving and sign them out when leaving. Upon arrival, deliver the child to his/her teacher. The person leaving and picking up the child/children must be certain that a staff member is aware of the child/children's arrival and departure.

In the event that your child cannot be controlled as suitable for group activity and has been advised in a parent conference that problems with behavior are a threat to the other children, the parent agrees to withdraw their child from Little Dreamer's Learning Center.

Children will not be allowed to bring toys or money from home unless requested by the child's teacher.

Liability
Once you have signed your child/children out on the sign in and out sheet. The L.D.LC will not accept liability for any accident that might occur. Therefore, if you linger in the center you should keep your child/children with you and in your care at all times. Do not allow the child children to run in the center.
