Reminders for Common Proof of Income Types:
- Paystubs: Must submit one month of paystubs (two if paid biweekly, four if paid weekly). Paystubs must be from the last three months with the employee name, pay period, and gross income visible.
- Letter of Employment: Must include how much the employee earns per hour and how many hours per week they work. Must be signed and dated by the employer.
- 2024 Tax Returns: Typically, only the first page of the 2024 1040 needs to be submitted. If you are self-employed, please include the Schedule C as well.
- Food Stamps (SNAP): Must include how much is received monthly with the recipient's name and date visible.
- Letter of Support: Must be fully filled out, signed, and dated.
Reminders for Proof of Residency:
Below is a list of acceptable documents to verify proof of TN Residency. If you do not have one of the below-mentioned documents, please reach out to the PANSC office to discuss other options.
- TN Driver’s License / State Issued ID
- Current Lease or Mortgage
- Utility Bill (electricity, internet, water, phone, etc.)
- Bank Statement
- Paystubs with Address
- Most Recent Tax Documents
*Must be dated within the past three months
Reminders for Bills:
Any bills should include the patient's name, the date of service, and the physician's name.
