Intake Form - New Athletes Born in Canada

PROOF OF RESIDENCY

For proofs of address, please submit three of the following:

1) BC Driver's License or BC ID

2) Car insurance matching DL/BCID (if you do no have a vehicle, upload TWO of the items below).

3) ONE OF (with address matching the above):

• Hydro Bill
• Cable/Satellite Bill
• Gas Bill
• Utility Bill
• Property Tax Bill
• Full rental agreement
• GST statement, income tax assessment, child tax credit

All proofs of address must be no older than 2 months.  Bills must show the entire full page bill, including name and address.

Note:  We cannot accept telephone/cellular bills, bank or credit card statements as acceptable proofs of address.

FAILURE TO COMPLY WITH SUBMITTING ALL NEW PLAYER DOCUMENTATION IN THE ABOVE MENTIONED FORMAT WILL RESULT IN THE ATHLETE NOT BEING PROVIDED WITH A HOCKEY ID NUMBER.

NOTE - EACH DOCUMENT CANNOT EXCEED 1000 KB OR 1 MB. If your file is larger, please compress it. ONLY PDF'S ACCEPTED!
